Use this form to report audio/visual (AV) issues experienced in a classroom, including problems with displays, projectors, audio systems, microphones, instructor computers, document cameras, control panels, or classroom connectivity.
This form is intended for reporting issues after class or when immediate technical assistance is not needed. Reports submitted through this form help Technology Services identify recurring issues, prioritize repairs, and improve the classroom experience for instructors and students.
